Information we collect

Appointment and contact forms

When you request an appointment or contact us through this site, we collect the information you provide — typically your name, email address, phone number, the reason for your visit and any message — along with your IP address, the page you submitted from and basic campaign data (for example, which ad brought you here). We use this to contact you about your appointment and to respond to your request.

Comments

When visitors leave comments on the site, we collect the data shown in the comments form, plus the visitor’s IP address and browser user-agent string to assist with spam detection. An anonymized string created from your email address (a hash) may be provided to the Gravatar service to determine whether you use it; the Gravatar privacy policy is at https://automattic.com/privacy/. After approval of your comment, your profile picture is visible to the public in the context of your comment.

Media

If you upload images to the website, avoid uploading images with embedded location data (EXIF GPS). Visitors to the website can download and extract any location data from images on the website.

SMS / text messaging

If you consent to receive SMS text messages from us, we may collect your mobile phone number, your opt-in consent status and the date and time of consent, and message delivery and interaction data.

Cookies

If you leave a comment, you may opt in to saving your name, email address and website in cookies for your convenience; these last one year. If you visit our login page, we set a temporary cookie to determine whether your browser accepts cookies; it contains no personal data and is discarded when you close your browser. When you log in, we set cookies to save your login information and screen display choices. Login cookies last two days and screen-options cookies last one year; selecting “Remember Me” keeps your login for two weeks. Logging out removes the login cookies. If you edit or publish an article, an additional cookie is saved that contains no personal data and simply indicates the post ID of the article you edited; it expires after one day.

Embedded content from other websites

Pages on this site may include embedded content (for example, YouTube videos or Google Maps). Embedded content from other websites behaves exactly as if you had visited the other website directly. Those websites may collect data about you, use cookies, embed additional third-party tracking and monitor your interaction with that embedded content, including tracking your interaction if you have an account and are logged in to that website.

How we share your data

If you request a password reset, your IP address will be included in the reset email. Visitor comments may be checked through an automated spam-detection service. Appointment requests may be shared with the scheduling and patient-communication platforms we use to book and confirm your visit.

SMS opt-in data — important notice

We will not share your opt-in to an SMS campaign with any third party for purposes unrelated to providing you with the services of that campaign. We may share your personal data, including your SMS opt-in or consent status, with third parties that help us provide our messaging services, including platform providers, phone companies and other vendors who assist us in the delivery of text messages. All of the above categories exclude text-messaging originator opt-in data and consent; this information will not be shared with any third parties.

How long we retain your data

If you leave a comment, the comment and its metadata are retained indefinitely so we can recognize and approve follow-up comments automatically. For users who register on the website (if any), we also store the personal information they provide in their user profile; all users can see, edit or delete their personal information at any time (except their username), and website administrators can also see and edit that information. Appointment-request records are retained as part of our business and patient records in accordance with applicable law.

Your rights over your data

If you have an account on this site, or have left comments, you can request an exported file of the personal data we hold about you, including any data you have provided to us. You can also request that we erase any personal data we hold about you. This does not include data we are obliged to keep for administrative, legal or security purposes.
