New patients of The Spine and Wellness Center can complete intake forms online through the secure ChiroTouch portal (intake.mychirotouch.com, clinic HLCC0002) before their first visit, or arrive 15 minutes early and complete them at the office. Bring photo ID, your insurance card and any recent imaging.
Save time at your first visit by completing your new-patient paperwork online through our secure ChiroTouch intake portal. It takes about ten minutes and means your first appointment starts with the doctor, not a clipboard.

Click below to fill out your intake forms before you arrive. If you have questions or would rather complete them in the office, just call us at (954) 341-2256 and arrive about 15 minutes early.
The intake portal is hosted by ChiroTouch and opens in a new tab. Your information is transmitted securely and handled under our HIPAA notice.
What to bring to your first visit
- Photo ID and your insurance card — we accept most major plans and car-accident (PIP) coverage
- Any recent imaging or reports (X-ray, MRI, CT) if you have them
- A list of current medications and treatments you have already tried
- Comfortable, loose clothing; most patients never need to change
What happens at your first two visits
Visit 1 — we measure (and usually begin care)
A conversation about your history and goals, then a hands-on structural exam with surface EMG and thermal scanning, posture and movement testing, and digital X-rays when indicated.
Visit 2 — full findings + plan
Dr. Jared Cohen, DC walks you through what we found in plain English and maps out a plan built for your structure. You decide, with no pressure — most patients have already started care on visit one.

Each visit is measured against the last so you can see progress, and your plan adapts to how you are responding through the 4R Method.
